A Condominium Questionnaire is a multipage document provided by the bank that we must fill out, for purposes of buyer financing. If the unit owner is refinancing, their bank may also require this document. If a unit owner is selling, the buyer\u2019s bank requires it. In order to have your Condominium Questionnaire completed, the unit owner or representing real estate agent must fill in the fields below and submit the information.<\/p>\n\n\n\n
Once this is done, a check for $75 will need to be made out to J. Butler Property Management, LLC and sent to:
P.O. Box 38, Andover, MA 01810<\/p>\n\n\n\n
Both items must be sent and received by our office before the questionnaire will be released. It is recommended that you submit the form and mail the payment a minimum of one week before the questionnaire is needed.<\/p>\n\n\n\n
